Program Information
Universal Healthcare IPA, Inc. is partnered with Sunrise Wellness Care to improve your overall health. Sunrise Wellness Care is a Medi-Cal , CalAIM Enhanced Care Management program operating in Kern County, California. The CalAIM Program does not replace your current health insurance, but rather works with your health insurance to improve your overall health and is available at NO COST to you.
The State of California and Managed Medi-Cal plans have established certain criteria to be eligible for the CAL-AIM Program:
- You must be enrolled in a managed Medi-Cal health plan
- Have certain chronic and/or mental health conditions
- Or have been recently hospitalized and/or frequent emergency room visits
Once you enroll, the Sunrise Wellness Care team will assist you with:
- Establishing a personalized care plan custom tailored to your specific needs
- Identifying doctors and scheduling appointments
- Assist in connecting you with community and social services for services such as transportation, housing, and nutritional needs
Again, if you meet the established criteria, the Sunrise Wellness Care program is available at no cost to you! You will not have to change your doctor, and enrollment in the Sunrise Wellness Care program does not affect your insurance.
